Feasibility of a Low-Cost Clinical Informatics and mHealth Program to Improve Diabetes Outcomes in Youth.

Despite multiple psychosocial barriers to self-management youth living with type 1 diabetes have few options for behavioral and social support. This lack of resources results in the worst outcomes of any age group. This use case demonstrates the novel and feasible use of patient-generated content and the integration of readily available technologies such as REDCap and SMS to push out tailored multimedia intervention content between clinic visits. The integration of a patient portal and clinician-facing screens add accountability and guidance for implementation in routine care. The model for and description of the Discovery Diabetes program may be used to support other programs without large infrastructure changes or costs.
